Vesicourethral anastomosis using V-Loc™ barbed suture during robot-assisted radical prostatectomy

چکیده
In Europe, localized prostate cancer (PCa) is increasingly treated by robot-assisted radical prostatectomy. This minimally invasive procedure is technically evolving, with benefits to both the patient and surgeon. One of the final steps of the operation is the formation of a watertight vesicourethral anastomosis (VUA). A suboptimal anastomosis can lead to serious consequences, including acute urinary leak, ileus, prolonged hospitalization and catheterization, urethral stricture, and urinary incontinence. It is therefore highly desirable to develop a simple, reproducible and dependable technique for VUA formation. We report a VUA formation technique using a unidirectional barbed suture (3-0 V-LocTM 180 absorbable suture, Covidien, Dublin, Ireland).

BACKGROUND The urethrovesical anastomosis is a particular challenging step of robot assisted radical prostatectomy (RARP). Failure to achieve a watertight anastomosis is associated with postoperative urinary leak and its consequences, which include paralytic ileus, prolonged catheterization, urinary peritonitis and possibly re-intervention. The bidirectional barbed suture is a new technology th...

PURPOSE V-Loc™180 (Covidien Healthcare, Mansfield, MA) is a new unidirectional barbed suture that may reduce loss of tension during a running closure. We evaluated the use of the barbed suture for urethrovesical anastomosis (UVA) during robotic assisted laparoscopic prostatectomy (RALP). Time to completion of UVA, post-operative anastomotic leak rate, and urinary incontinence were compared in p...
متن کاملThe Use of Unidirectional Barbed Suture for Urethrovesical Anastomosis during Robot-Assisted Radical Prostatectomy: A Systematic Review and Meta-Analysis of Efficacy and Safety
BACKGROUND Unidirectional barbed suture (UBS) has been widely used for surgery in recent years, especially for urethrovesical anastomosis (UVA) during robot-assisted radical prostatectomy (RARP). However, the efficacy and safety comparing it with conventional non-barbed suture (CS) for UVA is still controversial. AIMS The objective of this study is to assess the current evidence regarding the...

We report surgical management of a disrupted radical prostatectomy vesicourethral anastomosis after bleeding from undiagnosed hemophilia that required re-exploration, pudendal artery embolization, and urinary diversion with nephrostomy and surgical drains. After referral, the 4.5 cm vesicourethral anastomotic defect was reconstructed with a robotic-assisted abdomino-perineal approach. Intra-abd...
